SIGN-UPS HAVE BEGUN FOR THE FALL SEASON!

Students are encouraged to begin registering for the 2024 fall season.  To register for a fall sport, a student-athlete must return a completed PIAA Pre-Participation Physical Form.   These forms are available on the website under the TEAM REGISTRATION heading at the top and then click on PIAA REGISTRATION.  In addition to being online, these forms are available in the High School Office, Middle School Office and Athletic Office.  There are 5 pages to the form.  The first 4 pages must be filled out by the student-athlete and the parent/guardian.  Page 5 must be filled out by a physician, physician’s assistant or nurse practitioner (Authorized Medical Provider). 

According to PIAA rules, students must receive a physical exam from an Authorized Medical Provider on or after May 1 each year.  If a student-athlete is planning to attend the school-sponsored physical, a completed Pre-Participation Physical Evaluation form signed by the parent/guardian and the student must be returned no later than Monday, June 3.  PIAA Pre-Participation Physical Forms can be returned to the high school or middle school office or via USPS mail to the Athletic Office at 929 Lakeshore Drive, Leesport, PA  19533.  By having all forms by June 3, our Athletic Trainers and Team Physicians are able to review a students’ health history prior to the physical exam, which helps the process move along quickly and smoothly. 

On Tuesday, June 4, Schuylkill Valley School District will be holding fall sports physicals in the high school gymnasium area.  Physicals will begin at 6:00 pm.  All student-athletes must schedule a specific appointment time slot using the following link:

When arriving for physicals please enter the high school gymnasium lobby and check in at the registration table.  Once again this year, there is no cost for the athletic physicals.

HS football begins mandatory heat acclimation on Monday, August 5.  The golf season also begins on August 5.  To start the season on time, PIAA Pre-Participation Physical Forms must be turned in no later than Friday, August 2.

All other high school sports including cross-country, cheerleading, field hockey, boys’ and girls’ soccer, girls’ tennis, and girls’ volleyball begin the season on Monday, August 12.  To start the season on time, PIAA Pre-Participation Physical Forms must be turned in no later than Friday, August 9.

The junior high sports season includes cross-country, cheerleading, football, field hockey, boys’ and girls’ soccer, and girls’ volleyball begins on Monday, August 19.  To start the season on time, PIAA Pre-Participation Physical Forms must be turned in no later than Friday, August 16.
